Required Forms

Student Activity Advisors are required to submit the appropriate forms. 

• Each year, by September 10^th, the Purpose/Budget Form is due.  This form outlines what your plans are for the school year.  
• Every fundraiser requires the Fundraiser Request/Potential Income Form to be submitted PRIOR to conducting the fundraiser.
• When the approved fundraiser is completed, the Actual Income Form is required to be submitted within 30 days after fundraiser ends.
