Fairfield Local Schools uses FinalForms for student enrollment.  FinalForms is an online forms and data management system that guarantees compliance, distributes data, and reduces liability within our district.

FinalForms saves student data from sport-to-sport, club-to-club, and year-to-year, eliminating paperwork!

  • WELCOME TO FAIRFIELD LOCAL SCHOOLS!

    If you are new to our district, you will need to create a Parent Account in FinalForms.  Once this account is created, you can add your student(s).  Be sure to click on “New to District” if your child has never attended Fairfield.

    You will need to read/review and sign all forms in FinalForms.

    The first form you sign outlines all of the required documents for enrollment.  All of these documents (listed below) can be directly uploaded into FinalForms .  

    • student’s birth certificate
    • student’s social security card
    • student immunization records
    • proof of residency (utlity bill or rent/mortgage payment receipt)
    • parent’s drivers’ license
    • custody/court documents (if applicable)
    • student’s special education documents (if applicable)

  • Returning Students – Living in the Fairfield Local School District

    Student’s FinalForms accounts must be updated each school year.  Parents should sign into their accounts and update/sign each student’s forms before the first day of school.

  • Fairfield Local Schools accepts open enrolled students on a limited basis.  Students who are accepted are approved for one school year and must re-apply every year.

    Every April, the District holds a parent information meeting outlining the policies, rules and regulations of open enrollment.

    The open enrollment period opens up each year on April 1st.  Applications for the upcoming school year are due by June 1st.

    Information about the meeting date will be posted to our website news and Facebook page.

    The form can be emailed, faxed, or brought to the District Office.  
    If you are applying for the first time, the application must be approved prior to enrolling your student.

    Please DO NOT start FinalForms for your new student until you receive notification of approval.

  • Our preschool is only available to those students who reside in our school district.

    Each March/April an interest survey is shared publicly.  Your completion of this survey does not guarantee acceptance.

    We typically have more interest than we have openings.
